What Happened

Ohio State Highway Patrol officials are investigating a crash that killed a person at I-71 in West Salem, OH on April 11th, 2022. Police say the incident happened around 3:59 p.m. Investigators say a vehicle was involved in a one-vehicle crash that occurred under unclear circumstances. Emergency crews and police were at the scene shortly after and an approaching commercial truck reportedly struck the rear of a fire engine at the scene. It also struck a patrol vehicle and the earlier vehicle. The driver of the commercial truck, identified as 41-year-old Edyt A. Hernandez, was hospitalized with minor injuries. Lt. Philip M. Wigal, age 35, was killed while state trooper Steven B Hill, age 37, was hospitalized with non-life-threatening injuries.
