What Happened

Pennsylvania State Police are investigating a crash that killed one person involved at Route 422 in Slippery Rock Twp, PA on April 11th, 2025. Police are saying the incident happened around 3:57 p.m. According to investigators, a Chevrolet Silverado headed westbound is alleged to have collided with the rear of a Hyundai Santa Fe, causing the vehicle to overturn onto its side. Officials say the Santa Fe collided with a Smart Fortwo and a Honda Accord. Officials say 37-year-old Hyundai passenger, Kristen Ady, was fatally injured. The driver, 38-year-0ld John Ady was hospitalized with serious injured. Also inured was Smart Forwtwo operator, Rhonda Dam, who had less serious injuries. Authorities say the drivers of the Honda and Chevrolet were allegedly uninjured. Additional information was not available.
